Off Campus Studies Application This application and the various steps must be completed for a student to study off campus with an approved program. Student Name* First Last Student ID Number*Redeemer email address*Provide your Redeemer email address. All communication regarding your application will be sent to this email account. Major*List your major program. Current year level*Anticipated Year of Graduation*Off Campus Program*Please state the off-campus program that you are interested in. Term of Study*Please state the term (months) you wish to participate in the off-campus program. Student Comments*Briefly outline why you are interested in this off-campus study program and why you think you would be a good candidate for this program.Faculty Member Recommendation*Provide the email address of the faculty member in your major that can provide a recommendation for you for the off-campus program. This form will automatically be sent to the faculty member for their recommendation which will then be sent directly to the Registrar’s Office. Student Life Recommendation*Provide the email address of the student life staff member that can provide a recommendation for you for the off-campus program. This form will automatically be sent to them for their recommendation which will then be sent directly to the Registrar’s Office. NOTE: The student life recommendation must come from a staff member in the student life department: dean of students, associate dean of students, chaplain, residence life manager, residence life coordinator, or orientation coordinator. You are responsible for the full cost of the off-campus program you are participating in. The cost will be visible on your Student Account in Self Service. Full payment must be made prior to your departure. Financial Aid can be applied toward these costs and must be in place prior to your departure. However, where the host institution requires deposits or payment in advance of the student’s receipt of Financial Aid, you must pay Redeemer University out of your personal resources before the University will transmit any funds to the host institution. There is an administrative fee of $400 for the Redeemer in France Program (refer to the Academic Calendar for more details). This form will automatically be sent to the Business Office for approval, with their response sent directly to the Registrar’s Office.Consent*By consenting, I indicate my understanding of and agreement with the following: While studying off-campus, I agree to conduct myself in accordance with the ideals for which Redeemer stands, and to observe the rules governing student conduct. I acknowledge that I am responsible to ensure that a transcript is sent to Redeemer University Registrar’s Office after the program is completed (courses taken in an off-campus study program will appear on Redeemer University transcripts). I agree
